When you visit our office, you'll meet our dedicated professionals and our highly trained medical staff, who take time to get to know you and your situation. Your appointment will include an extensive exam in which we will make sure we truly understand your needs. A simple call to our office during regular hours is all it takes to set up an appointment. We'll find a time that fits your busy schedule. Remind us if you want to be seen at the Davison Office, the Lapeer Office, or the McLaren Wound Care Center in Flint.
What To Bring
Along with your insurance information and driver's license, please remember to bring any previous treatment information regarding your condition. If you had a previous x-ray or MRI, please bring these films or reports with you. If you have not had a previous x-ray and require one, we have an in office x-ray machine at both our Davison and Lapeer locations. If your appointment is regarding a second opinion or revisional procedure, previous consultation reports are beneficial. Also, be sure to print out and complete the 4 forms below.
Your time is valuable - so we thought of ways to make your visit even easier. Please fill out and print the following forms so we can expedite your first visit. If you can't, we'll have them waiting for you. Please arrive a few minutes early so you can complete your forms in our office prior to your scheduled appointment time.

Insurance and Payment Information
Payment is requested at the end of each office visit. We accept personal checks, VISA, MasterCard, cash, and most insurance plans. It is extremely important that you bring your insurance cards to your appointment so we may obtain the necessary and correct information to file all primary and secondary claims for you.
It is difficult to know all the specifics about each insurance plan. It is your responsibility to understand your health insurance deductible and co-pay and it is your responsibility to obtain referrals if you have an HMO.
It is necessary for you to check with your HMO for any special requirements. If you have an HMO that requires a referral from your Primary Care Physician (PCP), we ask that you call your PCP to verify that one has been completed. We ask that you bring a copy of the referral to your appointment. If you arrive without this information, you may be asked to reschedule your appointment until a referral has been completed.
We are participating physicians with Medicare and accept assignment. We will bill your secondary insurance when necessary.

We are Preferred Providers for BC/BS.
AUTO AND WORKERS COMPENSATION CLAIMS
We treat conditions arising from car accidents and injuries in the workplace. It is your responsibility to provide us with the appropriate active claim number and date of accident or injury.
We accept most insurance plans. Some of these include: Medicare, Blue Cross (PPO, Traditional, Advantage, MESSA, MI Child), Blue Care Network, Health Alliance Plan, Health Plus (HMO, PPO, Partners), Mclaren Insurance (Mclaren Health Plan and Advantage), United Health Care, HAP, Cigna, PPOM/Cofinity, Aetna and many others.
Any consultation reports from previous physcians can be beneficial. Other previous testing such as x-rays, CT scans or MRI reports are also beneficial.
Cancelling or Rescheduling
We understand that sometimes plans can change. If you need to cancel or change an appointment we ask that you give us 24 hours notice. This helps us keep our days organized so we can serve all of our patients as effectively as possible.
